placenta being highly vascular tissue , digital exam will provoke profuse bleeding and patient may even go for shock...so any 3rd trimester bleeding go for USG first..(after stabilizing of course ..)

even rectal examination is contraindicated in placenta praevia

Till some years ago double set up examination used to be performed in which patient would be taken to the operating room and a pelvic exam would be performed to ascertain the degree of placenta previa.With the widespread use of the ultrasound this is now obsolete. One never performs a pv in case of p.previa lest one causes torrential haemorrhage.
