An 8-month-old black female was brought to the emergency room with
shortness of breath, icterus and pallor under the tongue. The infant
measured below the 5th percentile for her age, despite weighing about the 80th percentile at birth. Examination of the peripheral smear indicates a hypochromic, microcytic anemia with some target cells and some teardrop-shaped erythrocytes. Hemoglobin electrophoresis reveals increased fetal hemoglobin (HbF) and double the normal amount of hemoglobin A2 (HbA2); there is very little hemoglobin A (HbA). There are no other hemoglobin bands.
