a 35-yr old patient, gravida 4, para 3, underwent an uncomplicated elective abortion 8
wks after her LMP. 3 days later she reports cramping pelvic pain and vaginal bleeding
that requires 2 pads every hour. Examination reveals that the uterus is at 8 weeks size
and softened. What is the next step in managing this patient?

A. Suction curettage
B. Oral Doxycycline
C. Obtain an upright X-ray abdomen
D. Hospitalize for hydration and ……
E. Serum HCG level

Ans is A. Suction curretage.
The lady has retained products of conception.

i was contemplating on that answer but you have mentioned she start to bleed 3 days after her elective abortion........isn`t it unlikely to develop late bleeding after an incomplete

I had seen so many patients like this one so let me explain few things. In this case if it was an incomplete abortion as you thought the uterus size should have been smaller then 8 weeks. Another explanation yes at time perforation of uterus show air under diaphragm. The diagnosis will be incomplete aboution and ERPC is the answer that is Suction curettage but only after Hospitalize for hydration and …… so my answer will be D.
Any coments on this merrk as it was very badly asked question?
