62 year old male comes with R and L leg weakness, urinary retention, decreased reflexes. O/E of prostate you find a firm nodular mass on DRE. On X-ray there is a lytic lesion involving T12/ L1. the bone scan shows multiple bone mets. LFTs normal. Rest of investigations are normal. What will u do for this pt? A. Radiotherapy spine B. Complete androgen ablation C. Radiotherapy D. Chemo E. TURP

Pt. shows advanz dse. bec. of obstructive sxs. may not req. tx., most are indolent cases but some are aggressive as this pt's. manifestation. Tx. depends on pt's. wishes. From this question, pt. needs radiotherapy of the spine.

I read that for stage IV prostate Ca, when the pt presents with symptoms of impending spinal cord compression, the treatment is complete androgen ablation. So I think the ans is B. This Q came in our exam so I dont have the definate answer, tell me what you think. thanks

even Kaplan mentions for widespread bone metastases....Order of treatment is Androgen ablation,Orchiectomy,Medical GNRH agonists & antiandrogens like flutamide
