Just came back . I was out buying some home stuff as I have a geust coming to stay over !!!
I feel like crap but need to get myself togetber and focus and try to study hard for the rest of the day !!!

unfortunately the last two days were awful lots of un expected problems!! And im a big time nervous, wich I could get rid of my nervousness and have a strong nerve ! I could barely read half of neurology which made me 2 days behind my scheduale. Im delaying uwsa until the last week , i subscribed for nbme 2 and will take it hopefully today or tommorrow once i feel better and relaxed. How many pages mtb are u doing per day and when is ur exam ?!

everything will be alright . just take a deep breath. I wont suggest you to take the nbme untill u feel relaxed and confident

Thank you . I need to do the same . I need to focus on reviewing UW and then mtb when Im tired . As majority of people here in forum say  one should master UW before the exam.

Im gonna finish Rheumatology from mtb the do nbme nodnodnod

Rest of the day will review some UW bolcks.

Tarsal tunnel syndrome (TTS), also known as posterior tibial neuralgia, is a compression neuropathy and painful foot condition in which the tibial nerve is compressed as it travels through the tarsal tunnel.[1] This tunnel is found along the inner leg behind the medial malleolus.

Inside the tunnel, the nerve splits into three different segments. One nerve (calcaneal) continues to the heel, the other two (medial and lateral plantar nerves) continue on to the bottom of the foot.

The tarsal tunnel is delineated by bone on the inside and the flexor retinaculum on the outside.

 


Depending on the area of entrapment, other areas can be affected. If the entrapment is high, the entire foot can be affected as varying branches of the tibial nerve can become involved.

Patients with TTS typically complain of numbness in the foot radiating to the big toe and the first 3 toes, pain, burning, electrical sensations,Pain worsens and spreads after standing for long periods; pain is worse with activity and is relieved by rest.

Plantar fascitis

Heel pain worst at the morning , improves with walking few steps . Point tenderness near calcaneus.

No correlation with the presence of heel spurs.

Meylodysplastic Syndrome 

SMEAR : Pelger Heut Cells : Bi-Lobic Nucleus neutrophils.

 

BM : hypercellular + parissan Bleu : Ringed Sidroblasts. 

Tx : Transfusion + Erythropioten + Lenalidomide.

how many hours you study each day.I really get tired after 6 hours.now a days I am studying less and less I think

Same here. Im doing like 8 hrs per day , after that I faint , I used to do better before. 

I don't know If I should postpone my exam or its just the fear inside me consuming my energy and enthusiasm.

Bronchoectasis Tx:

Chest physiotherapy + AB x ( possibly inhaled after defining specific responsible organism) +/- Surgical Resction.

ABPA Tx :

PO Steroids + PO Itraconazle ( Inhaled tx doesnt reach lesions

CF Tx :

ABx routinely after sputum culture ( Inhaled Aminoglycosides) + rhDNAse + Albeuterol + Vaccines ( Inf + Pneumo) +Lung Transplanation
