An asymptomatic 47-year-old man comes for a preemployment examination. He has never been hospitalized. He is a computer programmer, and he plays handball once weekly. His maternal grandmother had type 2 diabetes mellitus, and a paternal uncle had heart disease. The patient's blood pressure is 126/80 mm Hg. Examination shows no abnormalities. His total serum cholesterol level is 225 mg/dL. Which of the following is the most appropriate next step in management?

A
) Step 2 American Heart Association cardiac diet

B
) Serum lipid studies while fasting

C
) Exercise stress test

D
) Oral cholestyramine and niacin therapy

E
) Oral pravastatin therapy at bedtim

Please respond what would be the answer, Cause I think serum lipid cholestrol do not vary with / without fasting.

so, diet control would be appropriate

On Jun 19, 2013 - 8:56 PM, Ango responded:

I go with the fasting lipid profile first .

I just googled it for a proof and found this :

Serum lipid profile has now become almost a routine test. It is usually done in fasting state due to certain limitations in non-fasting serum sample. In the recent past efforts have been made to simplify blood sampling by replacing fasting lipid profile with non-fasting lipid profile. However, fasting specimen is preferred if cardiovascular risk assessment is based on total cholesterol, LDL cholesterol or non-HDL cholesterol. A lot has yet to be done in this area. Till then we have to believe in fasting lipid profile for assessment and management of cardiovascular disease.

and this make more sense .
