A 45 year old male has had malabsorbtion for the past year with diarrhea. He also has a polyarthritis and visual hallucinations. An abdominal CT scan reveals no masses, only generalized lymphadenopathy. On upper endoscopy, there are no esophageal or gastric lesions, but there are broad, flattened villi in the duodenum. Biopsies of the duondeum reveal numerous PAS-positive macrophages in the submucosa. Which of the following therapies is indicated for this patient:

A A gluten-free diet

B Corticosteroids

C Antibiotic therapy

D Local duodenal resection

E Antacids

Thats a good question I think it is Whipple's disease and the answer should be antibiotic therapy.

yes it is whipple disease which is treated by antibiotic therpay ....

can anyone tell me the causative organism of whipple disease and that of whipple like syndrome ??

I dont know about Whipple like syndrome- but I think the picture resembles TB

ok ... i am as you dont hear about whipple like syndrome but in the goljan audio i heared him said whipple like syndrome produced by mycobacterium avium intracellulare ... that is all what i know about it ...

Hey if I am not wrong he just says that the macrophages present in the Whipple's disease are similar to those that we would expect in mycobacterium avium intercellulare complex amd thats waht we can confuse this type of picture with and that its not an actual Whipple like syndrome

no i heard him said that mycobacterium avium peoduce symptoms similar to whipple disease that may be called whipple like syndrome ... and the PAS positive macrophages are present in many other cases so why he is interested in mentioning mycobacterium avium .... pleases hear the audio again and tell me about the result ... and then i will be sorry if i am wrong ...

RIA is right. MAC dosnot produce whippple like syndrome. there is not a synd. named this. But the picture MAC produces resembles that of whippelii... ı agree with Ria..

as in the case of hydralazine. it doesn't cause Lupus but clinical presentation of adverse effects resembles that of LUPUS.
