Cholecystitis and Biliary Colic
Constipation
Diverticular Disease
Endometriosis
Inflammatory Bowel Disease
Mesenteric Ischemia
Ovarian Cysts
Ovarian Torsion
Intussusception
Pelvic Inflammatory Disease

periumbilical pain followed by nausea, right lower quadrant (RLQ) pain, and vomiting.

well there is a long long list of DD for appendicitis, just a few more to add:
mittleschmirz
mesenteric lymphadednitis
Rt ureteric colic
hematoma or rectus abdominis
etc etc

Intially periumblical pain due to distension of appendix.it is a visceral pain. Later pain localises to right iliac fossa due to inflamation of parietal peritoneum,it is somatic pain. MIGRATORY PAIN is reliable symptom of acute appendicitis.
