a 12 yr girl with SOB , erthematous rash , swollen hip and knee joint , CXR shows enlarged heart + pulmonary edema , she develops CCF and dies on the 2nd day , what is the most likely recent medical history .
a, cyanosis with chest pain
b Jaundice ,
c , memingitis
d, pharyngitis ,
e skin infection .

what do you think is the cause of the disease .

cardiac failure , this is the question in the second block of the 2004 sample material on the ecfmg site .

Plz donot post the question with the information that it is from usmle CD. such things are remebered more. this will affect the score of others when they will attempt usmle cd. u can post them without giving any source

I agree its rheumatic heart disease....whats SOB???Is it a standard abbreviation used for the USMLE??
Answer is pharyngitis.
For mjl17...CCF is congestive cardiac failure.

SOB is shortness of breath, and afaik it is a commonly accepted medical abbreviation.

And I would agree with the pharyngitis causing acute rheumatic fever w. pancarditis, polyarthritis and erythema marginatum

Malaysian, "Rheumatic heart disease" and "Rheumatic Fever" are two different terminologies. Do not get confused between them.

Rheumatic heart disease is a chronic sequelae to cardiac component of rheumatic fever, probably due to hemodynamic changes, resulting in fibrosis, thickening of endocardium, fusion of cusps, etc.
