:idea:
A 58 yr old man is caught in the act of stealing for the third time. He has a conspicous history of street brawls,petty crimes and violent behaviour.
A close aunt revealed that this man was brought up in the most strict fashion and it is strange that despite all the punishment meted out he turned to a life of crime.
Upon being caught the man pleads "not guilty.
He is confined and during confinement he requests a psychiatric evaluation.The patient upon questioning volunteers complete ignorance about his family relations and even his aunt.The P/E reveals multiple somatic complaints of headache,tingling in upper extremities and abdominal cramps.
Assuming that this patient is not malingering which of the following should be the line of management:

1.) Psychotherapy along with counselling
2.) Pharmacotherapy with antidepressants/antipsychotics
3.) Refusal to give unnecessar drugs and Rx
4.) Assessment for suicidal risk
5.) Search for a "motive" or "personal gain"
6.) A complete,rigorous workup for his somatic complaints given his age.

i think he has an antisocial personality disorder ...

It is antisocial PD=answer A.

A clue Here:
"Not Malingering" does not necessarily rule out "Factitious disorder" does it.

It is Gansers syndrome. :!: I did not get it right though.

A prison inmate + h/o Stressors + Dissociative amnesia + Absence of Malingering + Multiple Somatic Complaintsv + The Corporal Punishment this guy was exposed to ,during childhood might actually have been child abuse [this history is often found in gansers] ................lots of clues but still a tough nut.Only one point favours Antisocial PD and that aint given as an option.

Rx for Gansers
Usually limited to recognition of the condition and the refusal to give unnecessary medications or to perform unneeded medical procedures
