To which axis this clinical condition belongs

The change in level of function of a 46 year old college professor ,who,during the past six weeks ,has not been able to go to work & spends his days in front of the TV ,unwashed & unshaven ,due to severe depression

Level of Function goes in axis 5.
The clinical diagnosis of (Major) depression would go in axis 1 but since it is the level of function that is asked it has to be 5.

Axis 2:Personality Disorder +/ Mental Retardation
Axis 3:General Medical Condition
Axis 4:Psychosocial Environment
Axis 5:Global Assessment of Function
