Whats the difference between Primary Biliary Cirrhosis and Primary Sclerosing Cholangitis? ( also what might the labs be?)

PBC
autoimmune progressive inflam and fibrosis ois and eventuf intrahepatic bile ducts leading to hepatitally cirrhosis. Asoc with other AI diseases and mostly women over 35. Osteoporosis.
very elevated AP, gamma-glutamyl transpeptidase
Less elevated bilirubin(even normal) and aminotransferases
elev. cholesterol
*anti-mitochondrial antibodies* vs inner memb of mitoch, also but less often this can be found in pt with chronic active hep.
High IgM's in general.
According to Merck, PBS is one of the best indications for transplant, with excellent results.

PSC
Intra and Extra-hepatic bile ducts are involved, prog inflam and fibrosis
Maybe AI, mostly young men and asoc with Ulcerative colitis.
High AP, less high aminotransferases, sometimes high bilirubin.
Liver transplant is the only cure here, too.

It is also said that PSC can eventuate into biliary cirrhosis. I would have thought vice versa since a football superstar died of PSC. Also ERCP is the gold standard for dxing PSC. Also see the words fibrosis and stricture for PSC but granulomatous destruction is used for PBC. Often only the alk phos is used for dx, along with AMA and liver bx for PBC. :shock:
