Hi out there...thanks very much for taking a look. I'm writing because I've encountered the 3rd 2004 remembered pdf on the USMILE Step 1 CD, which is COMPLETELY far more clinical than any other review resource I've seen, and looks like a Step 2 exam. Most of the questions deal with in-depth management and more esoteric diagnosis. Since I'm testing in a few weeks, I'm a little freaked. Has anyone seen these questions who has taken the Step 1? Is it comparable?

Well, here are 3 Which is an absolute contraindication to tocolytic therapy(??) for preterm labor? a) UTI b) <28 wk gestation c) chorioamnionitis d) uncontrolled DM e) vaginal bleeding from mild abuptio placentae What is contraindication for vaginal delivery after c-section? a) low segment transverse uterine incision b) breech c) clinically adequate pelvis d) placenta previa 80 yr old man with DVT and recent intracranial surgery for trauma. Next step in Mx is a) daily Doppler of LE b) IVC filter c) subQ hep d) tPA e) warfarin f) weekly V/Q scans Some of these are somewhat inferrable but seem a lot more management related than anything I've seen on other posts. Anyone who's taken the Step 1 who could give a thought would be REALLY appreciated!!!

yeah, i was so taken aback i actually went back to my cd to make sure that section was from a step 1 prep. anyone who's taken the exam have an opinion? has anyone else seen this on the usmile seen this and been surprised?

Yo These are step 2 questions. Step 1 deals with how well you know your basic sciences, and some BS inference. For example, what is the expected mechanism behind enterohemorrhagic E coli's aquisition of it's toxin... conjugation with shigella, and a plasmid... I think. Step 2 deals with patient care, and management. For example, 68 YOA M presents with chest pain. Px history is positive for a MI 6 months ago. What would be the next step in diagnosing a new MI... I think ultrasonography, but it's just a wild guess. These questions seem like Px care and management questions to me. Peace Lo

defnitiely not step I questions. There is very little clinically relevant stuff on step I. Even the clinical scenarios they come up with are quite contrived. Don't worry about these questions.
