A PREVIOUSLY HEALTHY 45 Y WOMAN HAS HAD FEVER AND PROGGRESSIVE CONFUSION OVER THE PAST 2 DAYS . SHE IS NOW UNABLE TO PERFORM ACTIVITIES OF DAILY LIVING,HER TEMP= IS 38C (100,4F) SHE IS ORIENTED ONLY IN PERSON. THERE IS NO RASH, NECK IS SUPPLE. CT SCAN SHOWS NO ABNORMALITIES.
CSF= LEUKOCYTES 20/mm3 GLU=45 Pr= 110 erythrocyte=1000mm3
WHICH IS DE MOST LIKELY DX????

A)BACTERIAL MENINGITIS
B)ACUTE OH INTOXICATION

I agree that its viral ..but im confused..may be u people r right HSV

CT would show temporal lobe abnormalities in HSV encephalitis. Why not cryptococal?

Hi,I think the answer is HSV as it is the MCC os encephalitis by far.Crypto would cause meningitis with neck stiffness esp in a IC pt.So I think its unlikely in this pt.

HI , NONE OF THESE OPTIONS GIVE AND INCREASE RBC COUNT IN CSF, SO I GUESS IT CAN BE A TRAUMATIC LUMBAR PUNCTURE. THE CSF SEEMS TO ME ALSO LIKE A VIRAL ENCEPHALITIS. I GUESS H. SIMPLE :?

Herpes does cause Haemorrhagic enceph .I forgot but thanks bronco for the reminder.So i think h is the best answer
