Hey everyone I have some contention with the answer that I found for this question in a book..so I am taking a poll to see what you all think should be the answer.

38 year woman presents with difficulty swallowing solid foods.She has gradually decreased her oral intake ,but doesnot report any weight loss. She hsa been taking medicines for heartburn on and off. No family history, no other medical history. Diagnosis is :

a. Scleroderma
b. Profuse oesophageal spasm
c.Benign peptic stricture
d.Achlasia
e.Squamous cell cancer

I can say that there is no cancer because the patient did not loose weight.
Also, no history for scleroderma but that's all I can comment for this question.

Hope we'll see an explanation for the correct answer.

The correct answer is benign peptic stricture because there is history of antacid which points to the acid peptic disease which if longstanding can cause complications like stricture formation, barrets esophagus in cases of prolong reflux.

i concur that the most appropriate answer is (a) scleroderma .... reasons being 1)no h/o weight loss rules out malignancy (2)she has h/o heart burn which can b attributed 2 incompetent LES ..common in PSS (3)it can't b eso. spasm ..coz theres no h/o severe retrosternel chest pain .(4) it doesn't look like achalasia as the presentation is a bit diff. like regurgitations& halitosis...(5) peptic strictures seems to b a good 1 but taking into consideration her sex & age .... i'll go with scleroderma though i won't b surprised if peptic stricture is the answer.shouldn't it b preceeded by severe & chronic acid peptic disease :roll: ....?? well do lemme know as well

I would think of it as a case of old peptic ulcer disease.... now presenting with the most common complication... Stricture formation...
Praveen i would disagree with you just because if it was Scleroderma they would make it more clear... there is no other significant history like dry eyes ....
Well, I Don't know the right answer but i think that PUD makes the most sense inthe ltd. info that we are provided....
