From DSM-IV on OCD:
The performance of rituals relieves the anxiety from the obsession temporarily. THINKING IS OFTEN MAGICAL (i.e. "My son won't have accidents if I stamp each foot 30 times.") and the patient is aware of this. It goes on to list intrusive thoughts, repetitive behavior, and depression; but does not explicitly state self-harm. I would think rather that the reason these patients have OCD is because they fear any harm and do what they do consciously in order to "wash the harm away." Perhaps any self-harm is unintentional and just a side-effect of what they are repetitively doing.
