I don't think anyone really knows what the numbers mean. Just from my score report, I figured out that failing is 1.5 SD below the mean. And the failing three digit score is set to equal 75. I think with some statistical adjustments, the three digit score is your "raw score." But it has been modified. That means you can in fact miss many many questions and still do quite well on the test.

Peeps two digit score DOES NOT equal percentage. It's the same score adjusted on their own scale. They say 182 is the same as 75. Again a 245 = 99 and a 260 = 99, so in USMLEs eyes those are the same, and in fact there may only be a 3-5 question difference between those two scores. It just depends on which ones you got right or wrong. Since it's already at the high end of the scale there is just not much room to improve, thus there is really no difference in those two scores. As for all the score between?? I don't know what it means, but it's not a %.
