Well I would say Number 1, because it says cancer and "other diseases", so, rhe physician must tell him the changes he most like to have in the short time like the change in teeth color, etc (HY Behavorial)

well I mean that you must tell that boy that the teeth or the finger tips are going to change in color (nicotine), instead telling him that he would probably get cancer. Because if he smokes and the parents hide the cigarrets, he will do it again if he has the opportunity. the fact is that you must prevent this talking to him (and obvious the parents too)
