For Mantoux to be positve u need good CMI.If the pt has miliary it means he hardly has any CMI left.So Mantoux -ve

B. Patients who are unable to mount a satisfactory Cell mediated immune response usually get miliary TB and therefore are unable to mount a response to PPD as well resulting in a negative Mantoux.
