What is better to be considered as a USCE:
1. observership or
2. volunteering or
3. work in private office as a medical assistant (taking vital signs and histories)?
I know externship is the best but I cannot found it, as well as a research, so please clarify for me from these 3 options what should I do?

working in a office is the best.. if you can..

the second best would be getting externships.. they are expensive, but they are work it.

Clerkship and Sub-I are medical student rotations in different years.

Both are medical student rotations. Not observership or externship.

In other words, if you have graduated medical school overseas, you cannot perform a Sub-I or (clinical) clerkship.

If you are looking for US direct patient care experience, you will be looking for an externship. An observership is just watching and doesn't count.
