Q.. several months after sustaining crushing injury to his arm, a patient complains bitterly about constant burning , agonizing pain in that arm, that doesnt respond to usual analgesics. Pain in his arm is aggravated by slightest stimulation of that area, such as rubbing of shirt sleeves. Arm is cold , cynotic and moist, but it is not swollen. Pulses at wrist are normal, and neurological functions of all three major nerves are intact.
Most appropriate to provide diagnostic confirmation of the nature of the problem and eventual therapy.


1. Angiogram and subclavian vein bypass
2. Cervical spine xray and Cervical rib resection
3. Doppler studies and arterial reconstruction
4.Doppler studies and fasciotomy
5.sympathetic block and surgical sympathectomy

faciotomy is for compartment syndrome which occurs soon after the # / after application of the cast.not several months later.
the case is classic for sudeck's dystrophy. the treatment i s sympathectomy.
