A 4 YEAR OLD BOY SUFFER A CAR ACCIDENT, HE WA UNRESTRAINED. IN EMG HE SHOWS MULTIPLE BRUISES OVER THE CHEST. HE DEVELOPS HYPOXIA. CXR REVEAL DIFFUSE INFILTRATES 4 HR LATER THE ACCIDENT. WHICH IS THE MOST PROLE DIAGNOSIS??

A) ARDS
B) PULMONARY CONTUSION
C) ASPIRATION PNEUMONIA
D) MIOCARDIAL INFARCTION
E) ESOPHAGEAL RUPTURE

why not pulmonary contusion?
please explain.

i think it is ARDS.becuse of the early presentation of case and xray findings
Pulmonary contusion usually present after 24-48 hrs of injury(especially xray finding)

BUT PULMONARY CONTUSION , AS KAPLAN QX SAYS IT CAN SHOW RIGHT AWAY AFTER TRRAUMA OR UP TO 48 HRS LATER.

HOW CAN WSE DIFFERENTIATE IT??? WITH THE DATA GIVEN IN THIS QUESTION. :shock:

Typical presentation of pulmonary contusion will be White lung on CXR
