hi friends, i just gave my step2 cs in atlanta yday. things about the exam: it was very basic &simple exam,great proctors,good patients,understandable accent.everything was just perfect.no worries about it.it started at 8 with orientation&demonstartion,then 5 cases then 30 min break,then 4 cases then 15 min break then 3 last cases.obviously timing is imp.this is how i went about it,1 min outside writing info&form a d/d then 8 mins of historyPAM HUGS FOSS&LIQORAAA,then 5 mins physical exam,2 mins counselling etc. this was ideal most cases i could close,but only the neuro cases it took me jjust half way thro counselling.anyway all that was just ideal,forgot certain thongs to do,thought abt it later,anyway did my best.another thing abt the patient note is my personal experience write the d/d&investigations first then physical exam &the history last cause when u start other wise,u take alot of time writing history,then d?D u cant think of many in the last minute hurry preparation:i guess practice&practice like the real exam is the only solution,usmle world cases are ideal nothing outside that was there,very basic.time ur practices,try to get hold of the kaplan cs [prep book if pos especially people with aproblem with enlgish,they also give u very good solution s how to answer certain challenging questions that the sp poses. i guess 2 week/max 3 week stiudy is enough.be realxed enjoy ur exam, any questions am happy to answer hope i pass [-o<

thanks for the advice! i hope we all pass this exam. i'll be taking my exam at the end of august. i have a question though. i understand that after the history and physical exam we have to tell the patient about our initial impression and the diagnostic work-ups. however, do we need to mention general therapeutic approach and give advices such as need for antibiotics, lifestyle modification, smoking cessation, etc?

definatelty abt lifestyle modifications like exercise sticking to medication but keep it brief best tell them we have an diet&exercise programme who can help u out or abt some social services program who can help

Hi Jil, thnx for sharing .I have few queries ab the xam if u can help me with them -- 1.do they give any card with findings written over it(like if u examine with otoscope they will give findings or with ophthamoscope??like written in OSCE book? 2.wht are the instruments kept in patient room ?besides phthalmoscope n otoscope?n 2 tuning forks n a torch ?? 3.i have never used ophthalmoscope n otoscope ,will tht be a problem? 4.did u have any teenage ,infant patients?? 5.i know tht u cannot discuss cases but can u tell us how many psychitric cases ,geriatric?gynae cases were there/? 6. did u get any patint who was in need of emergency ?then wht shud be done ,try to rlax him or history n examination? 7.are there any cases where u just hv to take history n no examination i asked for? 8. are we suposed to ask (formally) for PV/PR/breast examination or say tht we will do tht later?? i thnk thts a lot to ask for 1 go :P

thanks for the quick reply. i checked out the USMLEworld site and i can't afford the fee i hope mastering the osce book and the 40 cases here will be enough for the exam! 8-[

Jil thanks for the advises. Did you have any problem with time management. I mean I'm concerned about the pt notes. Did you finish on time all your notes? handwriting or typing? Can you start writing something in your interview paper before the 15 min of the encounter starts? Did you take PIPE? if you did, was it similar??

did you have any problem with time management. no not the way i told you i did it.practice with timing at home will be fine,typing Can you start writing something in your interview paper before the 15 min of the encounter starts? no dont write anything on the paper its an irregularity,they are strict even abt finishing your encounter&patient note,might as well loose a few points there than get an irregular behaviour on you Did you take PIPE? no chance fo pipe,but the people hwo took it said was the same

Dear Jil, I will be taking the CS later this month. I wish to ask if you wrote the "History of Presenting Illness" in the patient note in continous prose or in point form? Thanks

the point form is better according to me helps u rem everything &helps the examiner pick u the imp points quick makes it simpler

hi sorry no idea abt that exdactly but with the breaks it is a 7.5-8 hr exam maybe they might make it shorter hey sleep good ok allthe best buddy ull do good
