Is the green card or a working visa one of the most important criteria in selecting an applicant for residency? I know a person who has been "rejected" at an interview for this reason.

It is definitely not most important factor.

My program sponsored 20 visas this past season - about 2/3 of them were J-1b visas.

Was the person who was rejected asking for an H visa? Many programs still will not sponsor them because of the costs involved.

I'm not sure what kind of visa she was asking for (by the way, I didn't know that you ASK for a visa, I thought you take what they offer...). I know that she was expecting her Green Card and they weren't ok with that. In order to get an H1 visa you have to be real high quality, right?
Oh, and the ones that get a J1b visa, do they really go back to their home countries after they finish their residency?

Wouldn't that to be discrimination to give an H visa to someone who had higher scores? This program's decision for residents to receive an H or J visa was the date that they received their USMLE certification and the results of the Step 3 exam.

I have been working in three different IM residencies since 1988 - and I can count on one hand how many persons returned to their home country after finishing their residency/fellowship. One returned to work in Calcutta with the poor, one returned to Switzerland (who wouldn't), one returned to Hungary (after doing the waiver and getting the green card), and another one to India to open a hospital with their father. That's it.

Just remember that if you are starting residency on a J-1 visa - it's easier to get a fellowship as most fellowships won't sponsor an H visa.

The waiver positions that people have found have been in some decent places: Oklahoma City; Melbourne, FL; Arizona, Michigan, South Dakota, Wyoming.
