Hi guys! The best way to memorize urease positive organisms is using the pnemonic 'PUNCH' P-Proteus U-Ureaplasma N-Nocardia C-Cryptococcus H-Helicobacter Helicobacter is oxidase positive as well..

All strains of Nocardia are urease positive.. Its written in Jawetz Only some strains of Klebsiella are urease positive.. There's nothing different in AIDS.. its just that crypto meningitis is more common in AIDS.. Also, India Ink stain is not preferred much for diagnosis as it is not very sensitive (only about 50% positivity). Latex Particle Agglutination Test for Cryptococcal Antigen in serum or CSF is best (90% positivity). I've not read of urease test being used as a rapid method

Cryptococcal antigen testing is the most sensitive. Many hospital labs do not perform India Ink testing anymore and rely exclusively on the antigen. Our lab had three positive Cryptos this spring; two in CSF's within a week of each other and another in a blood culture from an IV drug user. The latter took four days to come up in the blood culture bottles. The organisms don't gram stain particularly well on direct smears and can fool some inexperienced micro people (as it did to one of our techs this spring, unfortunately). The bug showed readily in the India Ink prep and looked like a textbook picture. The antigen result was off the charts.
