Tamponade- pericardial window
LVD- Dobutamine+treat the cause of dysfnctn
Dialated Card Myopath- support+ Batista procedure
CHF- loop diuretic+positive inotropic agent+Dig consider transplant if no evidence of Pulm HTN
Hypertrophic Card Myop- transplant

Batista procedure- Dilated heart especially LV is trimmed off and diameter is reduced by cutting the diated noncontarctile part and sewing it back together.This reduces radius and hence increase wall tension(La place law) and force of contraction in systole.Fantastic concept came from Batista-from Brazil.

IABC-Intraaortic balloon counterpulsation-A catheter mounted with balloon is inserted through Rt femoral and connected to helium pump and ECG module machine.The tip of balloon radiologically sits at 1 cm diatal to origin if left subclavian artery.Helium is pumped into the balloon (30cc) during every diastole,and balloon inflates in thoracic aorta at every diatole.This reduces afterload (most important mechanism) and fills in coronaries by pushing the blood column and hence improving myocardial perfusion and hence helpful in cardiogenic shock.

He got it from his father (a carpenter). He came to our hospital, did on 3, 1 table death, 1 died post-op, 1 struggled for a while.

But he has reported better results at other centers, but still a controversial procedure.

Batista prcdr...Boston trials, Austin trials.. all negative results. Though only very sick pts were tried. SO no valid data yet.. but part of theory :-)

It is not a controversial procedure.It is established procedure ,however you are correct in interpreting the postop results.
Infact many further modifications have been made by various surgeons to it .The concept has eventually made many modification.
