a 20yrs old man is brought in ER by his girl friend within 1/2 hr of ingestion of acetaminophen tablets(nearly 8gm)...he presents with nausea and no other complaints...which would be an appropiate opinion..
A..acetaminophen level after 4hrs of ingestion
B..N-acetylcysteine

i'm sorry but i dont know the answer surely.just wantto clear my doubt.if anyone can help..

i guess after just 1/2 hr means gastric lavage or ipecac can help at this point....N-Acetyl cysteine is given only after measuring acetaminophen levels after 4 hrs ...(which if high..i don't remember the value)

according to kaplan notes
gastric emptying is sometimes useful within 1 hr of ingestion of ,but this does not delay the administration of NAC as a specific antidote

so i think gastric lavage +NAC

Its stage I overdose as only nausea is present + time <24 hrs.

I think that in < 1 hr - gastric lavage / ipecac + charcoal
If more than therapeutic level after serum analysis - after 4 hrs - administer NAC

But dpk - if no option of gastric lavage is given ; then probably correct answer would be NAC(B)

I saw similar Q too , they say always go for NAC.
