What is the 1st and 2nd drugs of choice for acute prostatitis?

"Trimethoprim" for it is basic and is attracted easily to the acidic enviroment of prostate sec.
if not effective i think "Quinolones"

Current practice is Quinolones first..coz of better tissue penetration and increased resistance of E. Coli to TMP-SMX. If refractory, consider Psedomonnas or Enterococcus as the cause, and treat with pseudomonas-PNC or aminoglycosides for Enterococcus.

Hey mdwannabe- I agree I was fairly sure the quinolones were 1st choice.
but unsure of the 2nd chice, and much surprised to her you say psuedomonas is sensitive to only pennicillin? {its a very opportunistic and virulent}Exotoxin A
*slime capsule,*ADP ribosyl transferase that inactivates protein synthesis,
*catalase, * pigments pyocynanin*, *endotoxin, * elastase.

Also some enterococcus is resistant to dinosaurs like gentamycin and streptomycin?

Sure..all ur concerns are valid... that is the accepted practice. If the bug is resistant you switch to something else. 3rd gen Cepha, Vanco.. etc..
cephalosporin or nafcillin G or Gram-positive cocci E. faecalis - ampicillin or amoxicillin, since TMP-SMX and quinolones are inactive against this guy. Anaerobic bacteria -amoxicillin/clavulanic or clinda.
