27 yrs old male with 1 week H/O SOB on exertion ,PND,swelling of feet.no H/O chest pain/palpitations .BP 90/60 ,PR 120 ,RR 24 ,JVD 8 cm .

O/E bibasilar crackles heard

Cardiac examin.....diffuse laterally displaced PMI
Normal S1 ,S2, S3

ECG is normal
Echocardiography will show..?

a. bicuspid aortic valve with stenosis
b. asymmetric septal hypertrophy
c. dyskinesia of left ventricular apex
d.MVP
E. diffuse hypokinesia and dilation of ventricles

H/O SOB on exertion ,PND,BP 90/60 ,PR 120 (Tachy)= explains LVF
swelling of feet,JVP -8 CM= explains RVF

no H/O chest pain/palpitations/NO murmur=rules out - bicuspid aortic valve with stenosis / asymmetric septal hypertrophy / dyskinesia of left ventricular apex ( will have ischaemic MR systolic murmur) / MVP

so answer (e) is correct.moreover he is typically young patient with 'quiet heart'- always rember funda- biventricular failure or cardiomyopathy.
