H/O SOB on exertion ,PND,BP 90/60 ,PR 120 (Tachy)= explains LVF swelling of feet,JVP -8 CM= explains RVF no H/O chest pain/palpitations/NO murmur=rules out - bicuspid aortic valve with stenosis / asymmetric septal hypertrophy / dyskinesia of left ventricular apex ( will have ischaemic MR systolic murmur) / MVP so answer (e) is correct.moreover he is typically young patient with 'quiet heart'- always rember funda- biventricular failure or cardiomyopathy. dmnz :wink:
