4 yr old male had motor vehicle accident .BP is normal ,PR normal,RR is 32 /min with grunting and retraction .Multiple bruises over the chest seen . ABG shows PH 7.38 ,PCO2 34 ,PO2 66 CXR after 4 hrs shows diffuse right sided infiltration . What is the diagnosis ??

i think of pulmonary contusion Can anyone diff with myocardial contusion ?

i agree with pul contusion- chest wall is pliable ,so no fracture occurs in 4 year old ,hence lung contusion is most common answer when no enzyme analysis and ECG findings are given.

Myocardial contusion usually presents after sternal fracture. There is no such indication in the case. Moreoever, "white-out" presentation of lung on chest xray confirms pulmonary contusion.
