Use both documents - write a letter to ECFMG and explain what is going on. Take the time to have the letter notarized and which name you would prefer to use. It's not uncommon. Good luck

Hi In most immigration forms there are fields for other names used, especially useful in your situation. So the immigration does take that into consideration. lf
