75 yr old man, with long standing COPD ( also h/o 60 pack/yr) has been bought by ambulance from nursing home. On his way to the hospital he recd Oxygen 10 l/min by mask. on admission he is unarousable and his ABG Most likely will be:
a. Pa O2 85 Pa CO2 65 PH 7.15
b. Pa O2 68 Pa CO2 50 PH 7.45
c.Pa O2 100 Pa CO2 25 PH 7.25
d.Pa O2 40 Pa CO2 85 PH 7.45
e.Pa O2 60 Pa CO2 40 PH 7.35

Buggers gave lot of oxygen to that poor old fellow; Co2 was washed away- you know that was the only thingy that kept him breathing by stimulating respiratory centre .

NEVER GIVE TOO MUCH OXYGEN TO OLD ,COPD PATIENT.

Now you know why he was unconcious.

But ans C presupposes hyperventilation in this case. Since pCO2 is 25. Ans is A.

Resp centers change their drive to O2 in sever COPD...ceneters depend on lower O2 concentration to drive the reflex...so once u give too much O2 u put pt in Resp failure resulting in hypoventilation, further CO2 retention, and CO2 narcosis.
